Critical Healthcare Forms – The 1095 series

Have you seen 1095-A, 1095-B or 1095-C Forms show up in your mailbox?

These forms are necessary in order to file accurate income tax returns for 2015.

What are these forms for?

  1. Form 1095-A: Prepared by the Marketplace Exchanges. Used to report information on all enrollments in qualified health plans in the individual market through the Exchange.
  2. Form 1095-B: Prepared by insurance companies. Used to provide proof of insurance to IRS.
  3. Form 1095-C: Prepared by large employers (50 or more full time or full time equivalent employees) who self-insure their employees.
